County Council Backs Plan to Tackle Delayed Discharges of Care

“Health and adult care services are working closely to tackle delayed transfers of care, which is bad for patients and costly for the NHS. Oxfordshire County Council believes the health service pilot could be a good way to tackle a long standing problem in Oxfordshire, and we will review progress over the winter.

The county council’s adult social care team has successfully reduced the number of delayed transfers and we will continue to work to ensure there is good, safe social care available when people are ready to leave hospital – preferably in their own home.

We know that discharging people from acute hospitals as soon as they are medically ready is the best thing as it reduces their chances of hospital infections and losing their independence.
